• All repairs are strictly C.O.D.
• Spare voice tube / ear cushions are available for on-site maintenance.
• In the interest of hygiene, ear cushions / voice tubes are automatically changed and charged for upon receipt by our service centre.
• If your unit is under warranty, please ensure a Proof of Purchase is sent with the unit.
• If you require consumables to be changed, a standard fee will be charged.
• If you require a quote prior to repair, please state that on the form.
• If you require a courier to collect, freight is for your account:
  R40-00 ex VAT One Way - Cape Town Area
  R55-00 ex VAT One Way - Outside Cape Town
  (one way is charged for warranty repairs)

Procedure

1. Download and complete the below form
2. Fax the completed form to 021-840 2250 (one way is charged for warranty repairs)
3. Pack your faulty items securely and include this form.
4. For delivery by courier, address the parcel to:
   Kalula Communication (Pty) Ltd
   Communications Centre, Urtel Crescent
   The Triangle, Somerset Mall
   Somerset West 7130
5. If you have so requested, we will arrange for Star Express Couriers to collect upon receipt of this form.
6. Retain proof of handover to Star Express or your preferred carrier.
7. All repairs are carried out on a COD basis. For your convenience, payment can be made by credit card.
